JOB DESCRIPTION

In the Health and Medical Care office, we have gathered the licensed staff of Welfare Gävle. We are now looking for two nurses for home care in Bomhus/Brynäs. Are you a trained nurse who enjoys a fun and flexible job? Great! Then we have the job for you. Together, we work for a holistic view of the patient and make Gävle safer.

As a nurse in home care, you will have a central role for our admitted patients. With us, you work both independently and in a team. You are responsible for the care of several patients admitted to home care. You work in a team around the patient together with physiotherapists, occupational therapists, doctors, relatives, and other nursing staff. As a nurse, you perform health and medical tasks in the patient's home, work preventively to promote the patient's health and well-being, and are responsible for medical assessments, documentation, and registration in various quality systems.

Your tasks include performing health and medical interventions, planning and collaborating around the admitted patients, and preventing care injuries. The most important task we have in home care is the collaboration with the palliative team of Region Gävleborg. Documentation is done in Treserva, planning is done in TES. Lifecare is used as communication between care providers. You will belong to the home care team working in Bomhus/Brynäs.

In your daily work, you will guide and delegate tasks to students and nursing staff. As a nurse, you have a significant responsibility to contribute to the continuous development and improvement of the operations.

Your working hours are mainly during the day, with evening and weekend shifts possible. Interviews may occur continuously during the application period.

QUALIFICATIONS

We are looking for you who are a licensed nurse and have worked as a nurse for at least two years. You should have good knowledge of the Swedish language in speaking, writing, and reading comprehension as the work requires documentation and other administrative tasks. A B driver's license for a manual transmission vehicle and driving experience is a requirement as you will be driving to patients' homes.

It is an advantage if you have experience in municipal health and medical care and if you are familiar with the systems: Treserva, TES, Lifecare, NPÖ, and Pascal.

To succeed in the role of a nurse, you are flexible and can quickly adapt and make decisions based on the patient and their needs. You work safely with patients and take initiative and responsibility for completing your tasks. You are structured and have the ability to see what needs to be prioritized, are efficient, and use resources to achieve the best results. Furthermore, you have good communication skills and adapt your language based on the recipient. You are confident in your role as a nurse and can distinguish between the matter and the person.

As you are the link to relatives and other nursing staff, you are skilled at creating and maintaining good relationships, both internally and externally. With good collaboration skills, you handle work situations with sound judgment and high integrity. You inspire trust, create confidence, and spread calm. You see opportunities in changes.
